I wish to participate in the exercise and training program offered by WIT Training. I am aware there are inherent risks in participating in a program of strenuous exercise. These risks include, but are not limited to:

• Falls that can result in serious or fatality.
• Injury or fatality due to negligence on the part of myself, my training partner, or other people around me.
• Injury or fatality due to improper use or failure of equipment.
• Injury or fatality due to a medical condition, whether known or unknown to me.
• I willingly assume full responsibility for the risks that I am exposing myself to and accept full responsibility for any injury or fatality that may result from participation in or attendance.

I have no physical impairments or illnesses that will endanger myself or others. I understand that WIT Training takes no responsibility for loss, theft or damage for any items brought into the Club, unless WIT Training is shown to have been negligent.

Release: In full consideration of the above-mentioned risks and hazards and in full consideration of the fact that I am willingly and voluntarily participating in the activities made available by WIT Training, I hereby waive, release and discharge WIT Training and its directors, employees and volunteers of any kind related to, arising from, or in any way connected with my participation in the exercise and training program.

This agreement shall be binding upon me, my successors, representatives, heirs, executors, assigns, or transferees. If any portion of this agreement is held invalid, I agree that the reminder of the agreement shall remain in full legal force and effect.

Images: Spectators and participants grant WIT Training and persons authorised by them permission to take pictures and movies of me and use them on their website, social media and other promotional and educational materials without compensation to me. In the event I choose not to allow use of such media I must inform WIT Training of this in writing.

Nutrition: I understand that the usage of any nutritional supplements is done under my own will.

Indemnification: I recognise that there is risk involved in the activities offered by WIT Training. Therefore, I accept financial responsibility for any injury that I may cause either to myself or to any other participants due to my negligence. I further agree to indemnify and hold harmless WIT Training, their directors, employees and volunteers from liability for the injury or fatalities of any person(s) and damage to property that may result from my negligent or intentional act or omission while participating in activities offered by WIT Training.

I have fully read and understand the foregoing assumptions of risk and release of liability, and I understand that by signing below it obligates me to indemnify the parties named for any liability for injury or fatality of any person and damage to property caused by my negligent or intentional act or omission. I understand that by signing this form I am waiving valuable legal rights.
